The registration should continue to match the service being operated
CQC registration records the provider, the regulated activities it may carry on and the locations and conditions that apply to that registration. When the service changes, the provider must establish whether the registration also needs to change before the new arrangement is implemented.
Changes may involve regulated activities, locations, conditions of registration, registered management arrangements, the service model or the legal entity carrying on the regulated activity.

Adding or removing a regulated activity changes the scope of registration
An existing provider that intends to begin carrying on a regulated activity not already included in its registration must apply to CQC for that activity and must not begin providing it until the application has been approved and the registration updated.
Where a regulated activity is no longer provided, the provider may also need to apply to remove it from the registration.
The proposed change should be checked against the actual service model, the locations from which the activity will be carried on, the Registered Manager arrangements and the Statement of Purpose.
Opening, closing or changing a location can require a registration application
CQC uses conditions of registration to identify the locations at or from which regulated activities may be carried on. Adding a new location or removing an existing location therefore requires the correct registration process.
The provider should establish which regulated activities will operate at the location, whether the premises and staffing arrangements are ready, who will manage the activity and whether related registration documents need to change.
Where the change forms part of a new service or major expansion, the registration application should remain aligned with operational readiness rather than being treated as a standalone form exercise.
Some changes require a condition to be varied or removed
Conditions can define where or how a regulated activity may be carried on and may also place particular restrictions or requirements on a provider's registration.
Where a provider wants a condition varied or removed, the application should identify the condition precisely and provide the evidence needed to support the requested change.
A condition relating to locations should not be confused with other conditions: adding or removing locations follows the specific location-change process.

A change to the regulated activities or locations managed by a Registered Manager can require a corresponding application or change to the manager's registration. Appointment of a new Registered Manager follows its own registration process.
The provider should therefore consider the provider application and manager application together so that responsibilities, regulated activities and locations remain consistent.
A change of legal entity is not simply an ordinary variation
CQC registration belongs to the registered provider. Where the legal entity carrying on the regulated activity changes, the existing registration does not simply transfer to the new entity.
Examples can include moving from an individual provider to a company, a transfer between companies, or another restructuring that changes the legal person responsible for the regulated activity.

A change in how the service operates can alter the registration position
Changes in delivery model should be assessed against the regulated activities and locations already registered. This is particularly important where a provider introduces a materially different service, moves activity between locations, adds remote or online delivery, changes the population served or introduces new clinical or care pathways.
The question is not whether the provider remains the same in name, but whether the service now being delivered still sits within the existing registration and Statement of Purpose.

Registration changes must be reflected consistently across the provider's evidence
CQC may require supporting information with an application to change registration. The Statement of Purpose should also remain accurate and reflect relevant changes to regulated activities, locations, management arrangements and the service provided.
