Statement of Purpose
Preparation and review of CQC Statements of Purpose so that the document accurately reflects the provider, regulated activities, locations, services and people the service is intended to support.
The Statement of Purpose is a statutory registration document
A registered provider must have a Statement of Purpose that contains the information required by Regulation 12 and Schedule 3 of the Care Quality Commission (Registration) Regulations 2009.
For new provider applications, CQC requires the Statement of Purpose as part of the supporting documentation. The information must be accurate, relevant to the proposed service and consistent with the application.
The Statement of Purpose remains a regulatory document after registration. Providers must keep it accurate as the service changes.
The document must describe the registered service accurately
Schedule 3 requires the Statement of Purpose to include the provider's aims and objectives, the services provided for the regulated activities, the range of service-user needs those services are intended to meet, provider and registered-manager details, the provider's legal status and details of the locations at which the services are carried on.
The document should identify the service clearly enough for the regulated activities, locations, provider structure and intended service-user groups to correspond with the registration application and the service that will operate in practice.

Regulated activities and locations must correspond with the application
The Statement of Purpose should identify the regulated activities carried on by the provider and the locations from which or at which those activities are provided, in accordance with the provider's registration position.
Inconsistencies between the Statement of Purpose, application forms, service description and premises information can create uncertainty about what the provider is applying to register.

The aims and objectives should describe the purpose of the service and the outcomes it intends to support. They should be specific to the provider rather than generic statements that could apply to any care or healthcare service.
The service description should explain the kinds of services provided and the needs the service is intended to meet. This should correspond with the regulated activities, staffing model, premises, referral or admission criteria and the way care or treatment will be delivered.

Provider and management information must remain current
The Statement of Purpose must contain the required information about the service provider and any registered manager, together with the provider's legal status and the locations included within the document.
The legal entity named in the Statement of Purpose should correspond with the person or organisation registered, or applying to register, to carry on the regulated activity.
Where management arrangements change, the provider should review whether the Statement of Purpose and related registration information also require amendment.
The Statement of Purpose should not conflict with the supporting evidence
CQC assesses the application as a whole. The Statement of Purpose should therefore be consistent with the provider application, registered-manager application where required, policies, governance arrangements, staffing structure, premises information and other supporting documents.
Differences in service-user groups, regulated activities, locations, management responsibilities or the description of the service may require clarification during the registration assessment.

The Statement of Purpose must be kept accurate as the service changes
A provider's Statement of Purpose should continue to reflect the registered service. Changes to locations, regulated activities, service-user groups, management arrangements or the way the service operates may require the document to be updated.
An updated Statement of Purpose does not itself alter CQC registration. Where a change requires an application to vary registration, the appropriate registration process must also be completed.
