48‑Hour On‑Site Intervention
Oxara Consulting provides urgent, director-led on-site assessment for care services facing acute operational or regulatory pressure. Subject to conflict clearance and availability, a named director can be deployed across England within 48 hours of an accepted instruction. We triangulate the available evidence across people's experience, staff feedback, observation, records, processes and outcomes, establish immediate action ownership and provide an initial risk-prioritised plan before leaving site.
Early assessment establishes the immediate risks, evidence requirements and actions that need clear ownership.
Within 48 hours of an accepted instruction, subject to availability and location, a named Oxara director is on site. Where appropriate and with consent, they speak with staff, people using the service and representatives, review the records relevant to the presenting concern, and walk the service. Findings are evidenced using Oxara's structured evidence-review methodology, mapped to the relevant regulations and CQC quality statements, not assumed. The work is led by the attending director throughout, not delegated to a junior team.
When to instruct Oxara
Providers may instruct Oxara for 48-hour intervention where:
- a service is showing early signs of operational or governance slippage;
- an inspection is imminent and the provider needs an independent assessment of whether documented systems are operating in practice;
- leadership has changed suddenly or is otherwise absent from day-to-day oversight;
- a safeguarding or clinical incident requires an immediate, independent operational review;
- staff, resident or relative concerns are accumulating faster than the service can respond;
- a commissioner or local authority has escalated concerns, or a placement suspension is threatened;
- governance or escalation reporting has broken down and needs an independent reset;
- a second opinion is needed before committing to a longer engagement.
If a service is under acute pressure, or attendance may be needed at short notice, a director can discuss the position with you.

Before leaving site, Oxara ordinarily provides:
- an immediate risk and action summary;
- clear ownership of urgent actions;
- a leadership briefing on material findings;
- a preliminary action plan sequenced by safety and regulatory risk;
- identification of matters requiring prompt statutory escalation.
Follow-up reporting
Where agreed, subsequent outputs may include:
- a reconciled documentation and evidence-gap review;
- a written governance and leadership assessment;
- a structured summary of stakeholder evidence, gathered and handled with appropriate consent and confidentiality;
- a detailed remediation plan;
- recommendations for further specialist or embedded support.
The scope and reporting format are agreed at the outset of the instruction.
Availability and the 48-hour deployment target
Oxara targets on-site deployment within 48 elapsed hours of accepting an instruction. Deployment is subject to conflict clearance, director availability, safe site access and receipt of the information required to scope the intervention.
The named attending director, deployment window, initial scope and any travel arrangements are confirmed in writing before the instruction begins.
Evidence reviewed on site
The intervention is risk-led rather than a fixed checklist exercise. Evidence is selected according to the presenting concern, service type and agreed scope.

Scope and exclusions
The intervention is a rapid, risk-led assessment and stabilisation service. It is not a full inspection, organisation-wide audit, statutory safeguarding enquiry or guarantee that every underlying issue will be identified during the initial visit.
What happens after the visit
Following departure, Oxara finalises any agreed written findings, requests further evidence where needed, and confirms ownership of the action plan with provider leadership. Where the presenting issue requires longer support, this may be scoped as embedded leadership, governance strengthening, enforcement support or a referral to the provider's legal advisers. A follow-up call is agreed as part of the initial instruction.
Reporting scope and professional boundaries
Oxara provides independent clinical, operational and governance assessment. It does not provide legal advice, determine appeal or litigation strategy, or represent the provider before a court, tribunal or regulator.
The intervention does not transfer the registered provider's legal responsibilities to Oxara, replace the registered manager or nominated individual, or substitute for emergency, safeguarding, commissioning or regulatory action. Operational decisions remain with the provider unless a separate interim leadership appointment is expressly agreed.
Where an immediate or material safeguarding concern is identified, Oxara will raise it with the authorised provider representative and support prompt use of the applicable local safeguarding, emergency and regulatory notification routes. The engagement does not replace the provider's statutory duties or the responsibilities of safeguarding authorities. Oxara's assessment is independent in professional judgement but is not a statutory safeguarding enquiry.
All engagements are subject to formal conflict checks and handled with strict professional confidentiality.
Director-led delivery
A named Oxara director leads the on-site intervention and remains professionally accountable for the findings and action plan. Additional specialist input may be introduced where required and agreed, but core assessment, on-site judgement and reporting are not delegated wholesale to a junior team.
Instruction leadership
The attending director is selected according to the presenting risk, service type, clinical complexity and operational context of the instruction.
